What to Expect at Registration
When you arrive at Lake Ridge Ambulatory Surgery Center, our registration team will help you complete the check-in process and ensure everything is ready for your procedure.
During registration, you can expect to:
- Check in at the registration desk.
- Verify your personal information, including your name, date of birth, address, phone number, emergency contact, and insurance information.
- Present a valid photo ID and your insurance card so we can verify your information and make copies for your medical record.
- Review and sign required registration forms.
- Pay any required patient responsibility, including copays, coinsurance, deductibles, or any outstanding balance due at the time of service.
- Provide a copy of your Advance Directive or Medical Power of Attorney, if you have one.
- Confirm your transportation arrangements by verifying the name and contact information of the responsible adult who will drive you home and receive your discharge instructions. Your responsible adult must be present at check in and will be asked to sign required documentation and provide a current contact phone number before your procedure begins.
- Ask any registration or billing questions you may have before proceeding to the clinical area.
Once your registration is complete, a member of our clinical team will escort you to the pre-operative (prep) area, where your nursing assessment and surgical preparations will begin.
Please Remember to Bring
- A valid photo ID
- Your insurance card
- A debit card, credit card, or cash for any required copay or patient balance (personal checks are not accepted)
- A copy of your Advance Directive or Medical Power of Attorney, if applicable
